Description - Assess patient needs in both inpatient and outpatient environments and craft tailored therapeutic plans

- Work alongside nurses, doctors, case managers, and other rehabilitation specialists to achieve optimal patient outcomes
- Provide education to patients and families about treatment goals, discharge processes, and at-home rehabilitation exercises
- Maintain comprehensive records of evaluations, treatments, and progress in line with healthcare regulations
- Engage actively in team rounds and interdisciplinary meetings for coherent patient care
Education - Doctor of Physical Therapy (DPT) or the equivalent degree from an accredited institution
Certifications - Current state licensure as a Physical Therapist
- CPR certification is preferred
