Job Description

ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician (PCP) to provide care for patients with acute and chronic illnesses, primarily older adults. The PCP is accountable for outcomes, clinical care, and cost-effectiveness for a panel of up to 450 patients, acting as a health coach to help patients set and achieve health goals. Responsibilities include assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation. The PCP leads a care team and collaborates with hospitalists, transitional care teams, and other healthcare professionals. The role requires adherence to departmental goals, regulatory compliance, and quality patient care standards. The PCP will also participate in risk and quality management programs, clinical meetings, and center management activities, including recruiting patients and providers. The position demands availability and accessibility to patients, evidence-based medicine practice, leadership development, quality improvement, and self-care. Proficiency in Microsoft Office and willingness to travel locally and nationally for training and recruiting are required. Education and experience criteria include an MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty, state medical license, board eligibility or certification, DEA number, and Basic Life Support certification within 90 days of employment.
