With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of the harm perpetuated by centuries of racist, socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community and healthcare systems in partnership with community, faith-based and health care organizations. CHECW's works focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well being of New Yorkers. CHECW is comprised of the Bureau of Bronx Neighborhood Health, the Bureau of Brooklyn Neighborhood Health, the Bureau of Harlem Neighborhood Health, the Bureau of Chronic Disease Prevention, the Bureau of Health Equity Capacity Building , the Bureau of Equitable Health Systems and the Bureau of Finance, Administration and Services. The division's Deputy Commissioner also serves as the Agency's Chief Medical Officer and oversees Race to Justice, the Agency's internal reform effort to help our staff learn what they can do to better racial health gaps and improve health outcomes for all New Yorkers. CHECW sits under the Agency's Chief Medical Officer. The Bureau of Bronx Neighborhood Health seeks to hire a Action Center Coordinator, Tremont Neighborhood Health . The action Center Coordinator will support the day to day Bureau operations and coordinate neighborhood engagement efforts aimed at improving population health and better meeting the needs of New Yorkers. Duties Will Include But Not Be Limited To Plan and support all community engagement activities and events at the Action Center. Lead the new Bronx Electronic-Health Hub Project. Convene an Action Center Advisory board to inform program planning and bureau priorities. Represent the bureau at Bronx Community Board meetings. Coordinate health education and information trainings, workshop, and presentations. Make recommendations on bureau policies and procedures. Manage conference room schedules, activities, equipment, policies, and protocols. Maintain and regularly update referral and educational resources. Provide community referrals. Respond to all public inquiries via telephone, text, and email. Assist with Bronx Bureau operations and administrative tasks. Participate in internal and external community meetings and events. Manage and track programmatic inventory. Maintain records and provide programmatic and building activity reports. Supervise community health workers and/or interns. Participate in public health emergency response as needed and support the agency in community engagement during an emergency. Other duties as assigned. Preferred Skills Ability to prioritize and handle multiple assignments. Experience in community engagement and working with community residents. Interest in addressing health inequities and disparities. Ability to work closely with all levels of staff inside and outside the Agency. Experience working with diverse groups and community-based organization stakeholders. Ability to work independently and exercise initiatives to accomplish tasks and solve problems. Minimum Qualifications A baccalaureate degree from an accredited college and two years of experience in community work or community centered activities in an area related to the duties described above; or High school graduation or equivalent and six years of experience in community work or community centered activities in an area related to the duties as described above; or Education and/or experience which is equivalent to "1" or "2" above. However, all candidates must have at least one year of experience as described in "1" above. Residency Requirement New York City residency is generally required within 90 days of appointment. However, City Employees in certain titles who have worked for the City for 2 continuous years may also be eligible to reside in Nassau, Suffolk, Putnam, Westchester, Rockland, or Orange County.
