Our ideal candidate is a courteous, efficient, and patient-focused professional who can provide exceptional care to a special needs patient population and their caregivers in a dynamic outpatient setting.

Licensed Practical Nurse (LPN)
Location: Multiple FQHC Location Brooklyn
Schedule: Monday – Friday, 8:30 AM – 5:00 PM (40 hours per week)
Employment Type: Full-Time

Key Responsibilities

Coordinate and prepare daily assignments for nursing staff
Assist in implementing systems for daily medical and nursing operations, in collaboration with the Director of Nursing
Participate as a key member of the Patient-Centered Medical Home (PCMH) Care Team , attending daily huddles and practice team meetings
Ensure compliance with all PCMH initiatives and standards
Provide high-quality, patient-centered nursing care across multiple clinic locations

Active Licensed Practical Nurse (LPN) license in New York State

Valid NYS Registration Certificate
BLS Certification (Basic Life Support)
Experience working with special needs patient populations is preferred
Strong communication and teamwork skills
Ability to adapt and work efficiently in multiple clinic environments
