Location: Boca Circle Rehabilitation Center, Boca Raton, FL
Shift: Full-time, 8:30 AM - 5:00 PM
Salary: $26 per hour
Job Description:
Boca Circle Rehabilitation Center is seeking a dedicated Social Worker Assistant to support our Social Services team. The ideal candidate will assist in planning, developing, and implementing social services programs to enhance the well-being of our residents.
Responsibilities:
Assist the Social Services Director/Consultant/Administrator in organizing and directing social services programs.
Collaborate with administration, medical, nursing, and other departments in planning social services.
Participate in discharge planning, care plan development, and resident assessments.
Encourage resident and family participation in care plan meetings.
Educate staff on Resident’s Rights and promote dignity and respect for all residents.
Conduct Advance Care Planning upon admission and ensure Advance Directives are regularly reviewed.
Inform and educate residents and their representatives about healthcare options and end-of-life planning.
Ensure staff members are aware of resident code status and end-of-life preferences.
Requirements:
High School diploma
Basic computer skills
CPR certified and trained in First Aid
Prior experience in skilled nursing, assisted living, long-term care, or healthcare settings
Experience with Resident Care Planning, particularly for elderly or medically complex residents
Minimum 1 year of experience in a skilled nursing home
